Office Billing Procedures

At the time of your scheduled appointment, we ask that you bring your most current insurance card.
Payment is due at time of service and is collected at check-in. Included fees are co-pays, co-insurance, deductibles and any past due balance on your account. Our office accepts cash, check, Visa, MasterCard, Discover and American Express.

Appointment reminders are phoned 48 hours in advance. As a courtesy, we ask that you notify our office within 24 hours of your scheduled appointment if you need to cancel and reschedule. This enables us to use that appointment time for another patient. A $25 missed appointment fee will be charged if our office is not notified within 24 hours.
